Persistent Heartburn Could be a Sign of GERD – Dubai Surgeon Explains When to Seek Help
Heartburn is one of the most common digestive complaints worldwide, but persistent heartburn or severe acid reflux symptoms may indicate a deeper medical issue. Many people dismiss it as “just indigestion,” yet ongoing symptoms can signal gastroesophageal reflux disease (GERD), esophagitis, or, in rare cases, precancerous changes in the oesophagus.
As a Consultant General & Laparoscopic Surgeon at Fakeeh University Hospital in Dubai, I often see patients who have tolerated reflux symptoms for months before seeking help. The encouraging news is that most reflux-related conditions are highly treatable, particularly when diagnosed early.
Understanding Heartburn, Acid Reflux and Dysphagia
Heartburn occurs when stomach acid flows backward into the esophagus, causing a burning sensation in the chest or throat. This can happen for many reasons, including:
- A weak lower esophageal sphincter (LES)
- Obesity
- Smoking or excessive alcohol
- Certain medications
- Stress, poor sleep, and dietary habits
When the valve between the stomach and oesophagus fails to close properly, acid escapes upward and irritates the oesophageal lining which can lead to difficulty in swallowing, this is called dysphagia.
Symptoms You Should Never Ignore
Occasional heartburn is normal but persistent or worsening symptoms may indicate a more serious condition. You should seek medical evaluation if you experience:
- Heartburn more than twice a week
- Difficulty or painful swallowing (dysphagia)
- Food feeling “stuck” while swallowing
- Unexplained weight loss
- Chest pain not related to exercise
These symptoms may indicate GERD, oesophagitis, or structural problems that require evaluation by a specialist in reflux disorders. If you are unsure,

Lifestyle Changes That Can improve Heartburn
Simple lifestyle adjustments can significantly reduce reflux symptoms:
- Eat smaller, more frequent meals
- Avoid heavy meals near bedtime
- Reduce spicy, fatty, or acidic foods
- Maintain a healthy weight
- Quit smoking
- Limit alcohol intake
- Elevate the head of your bed when sleeping
If symptoms persist despite these changes, a surgical assessment is essential.
When Medication Isn’t Enough
If heartburn continues despite acid-reducing medications, or if symptoms return quickly, your LES valve maybe weakened. Further evaluation can determine whether:
- Reflux is severe
- The oesophagus is inflamed
- The LES valve is failing
- There is a structural issue needing treatment
For some patients, laparoscopic anti-reflux surgery, including Nissen Fundoplication or Toupet Fundoplication can provide long-term or permanent relief.
Take home message
Persistent heartburn is more than a daily convenience; it can indicate a serious underlying condition. With proper evaluation and a personalised treatment plan, most patients experience significant symptom improvement and better quality of life.

Heartburn & Acid Reflux Symptom FAQ
- Why hasn’t my medication worked?
If lifestyle changes and medications haven’t helped, your LES valve may have weakened. A surgical assessment can help create a targeted treatment plan.
- Why do I have a constant acid taste in my mouth?
This suggests stomach acid is escaping upward. Over time, this can damage the oesophagus. Proper evaluation is important, especially if medications have not improved your symptoms.
- Why does it feel like food is stuck when I eat?
Chronic reflux can inflame or narrow oesophagus. This symptom requires prompt investigation, usually via endoscopy.
